To enroll your child, the following items are required:

• Completed enrollment packet
   
• State-issued birth certificate
   
• Physical and immunization record
   
• Proof of residency
   
• Emergency contacts and authorized pick-up list
   

• Toilet training (age-dependent)
   
• Optional extended evening care based on Block Scheduling availability
   

• Follow center policies and procedures
   
• Maintain open communication with teachers and administration
   
• Support a positive, faith-based learning environment
